If you already have a One Health Pass profile and have chosen Detoxicare Molecular Diagnostics Laboratory as your testing laboratory, you can enter your OSS QR Code and Last Name to get the details of your registration.

Important reminder: Detoxicare Molecular Diagnostics Laboratory will only be able to schedule your swab test if your One Health Pass profile is completely verified.
